Although I have never had a wheel come off, going from general online consensus I would definitely recommend having a sleeve machined up. It seems that this is done mainly for peace of mind but would definitely help if nuts loosened. This may take abit of doing as it will have to neck down so that id becomes od after the hub (assuming the wheels used were originally hubcentric).
I thought about this abit before I bought my adapter spacers and concluded that as long as;
- the adapters were hubcentric
- the adapters were wheelcentric
- the lug nuts were torqued to spec and correct shape (you use original wheel nuts to fasten adapters to hubs, be sure that they match the adapter recesses)
It should provide no greater load on the hub than a wheel of the equivalent width with x amount greater negative offset, where x is the width of the adapter.

Good luck but do this at your own risk. There is a reason they are not legal in Victoria unless they could be dealer optioned / came from factory on a vehicle (which they were on 240s)
