Cost can be a big influence here (not neccessarily in this case, as I dont know).
A lot of the more common pistons that you may be referring to are coming out of China - not that there is necessarily anything wrong with that - where labour is inexpensive and triple-checking what you've just had made is a necessity.
Sure, they have everything you've spec'd in your build, but it is hard to tell exactly what they're made from, how they're made and how the treatment process works.
We spec'd a custom 16V set in the last couple of weeks. Quite expensive, but made to order and we know exactly what they're made from and have a complete understanding of their manufacturing process - this can be where some of the costs come from.
Typically, production of anything in small volumes is going to be expensive. When you add a completely customised part into that mix, costs naturally add up too.
